KAJIAN AKTIVITAS DAN MEKANISME KERJA EKSTRAK
DAN FRAKSI BUAH KRANGEAN (Litsea cubeba (Lour.) Pers.)
SEBAGAI ANTIDIABETES

Diabetes mellitus (DM) adalah kelompok gangguan metabolik yang disebabkan
karena pankreas tidak memproduksi insulin atau tubuh tidak dapat menggunakan
insulin yang diproduksi secara efektif. Insulin merupakan hormon yang mengatur
keseimbangan gula darah didalam tubuh agar tidak terjadi peningkatan kadar
glukosa dalam darah.
Peningkatan jumlah penderita DM terus meningkat di seluruh dunia. Berdasarkan
data WHO, Indonesia menduduki peringkat ke 2 terbesar di negara anggota
SEARO (Southearn Asian Pasific) dengan persentase kematian akibat diabetes
sebesar 6%. Prevalensi nasional DM tahun 2018 berdasarkan konsesus PERKENI
2015 meningkat menjadi 10,9% dari konsensus PERKENI 2011 sebesar 85%.
Pada saat ini DM ditangani dengan obat-obatan sintetik yang memiliki efek
samping yang relatif tinggi serta harga yang lebih mahal. Selain obat-obat
konvensional, saat ini juga terlihat peningkatan minat masyarakat terhadap obat
tradisional, diantaranya dikarenakan efektivitas yang menjanjikan, efek samping
yang lebih sedikit serta biaya pengobatan yang lebih murah.
Litsea cubeba (Lour.) Pers. atau krangean merupakan tumbuhan yang banyak
terdapat di dataran tinggi. Komponen minyak dari krangean banyak dimanfaatkan
sebagai pengobatan. Buah krangean sendiri secara empiris telah digunakan
sebagai pengobatan diabetes, dengan menggunakan satu buah krangean mentah
dikunyah 2 kali sehari selama 4-6 minggu. Namun demikian, masih diperlukan
penyelidikan lanjut untuk mengetahui golongan senyawa apakah yang
bertanggung jawab terhadap aktivitas antidiabetes tersebut. Lebih jauh, juga perlu
diselidiki bagaimana mekanisme senyawa tersebut dalam menurunkan kadar
glukosa darah.
Penelitian ini bertujuan untuk menemukan aktivitas dan mengkaji mekanisme dari
tumbuhan krangean yang diekstraksi menggunakan Supercritic fluid extraction
(SFE), dengan pelarut etanol serta difraksinasi dengan menggunakan pelarut n-
heksana, etil asetat dan air. Penggunaan SFE sendiri bertujuan untuk memisahkan
komponen minyak dengan senyawa kimia lain yang terkandung dalam buah

krangean. Untuk mencapai tujuan ini, rangkaian penelitian dirancang sebagai
berikut: ekstraksi total buah krangean dengan menggunakan pelarut etanol,
ekstraksi dengan menggunakan SFE, ekstraksi residu SFE dengan menggunakan
pelarut etanol, fraksinasi ekstrak residu buah krangean dengan menggunakan
pelarut n-heksana dan etil asetat. Pengujian aktivitas antidiabetes in vivo dari
ekstrak dan fraksi dengan metode uji toleransi glukosa oral, metode induksi
aloksan dan metode induksi resistensi insulin dengan makanan tinggi lemak-
karbohidrat pada hewan uji (seluruh pengujian in vivo dilakukan dengan dosis
bertingkat, ekstrak pada dosis 140 mg/kgBB, 280 mg/kgBB dan 560 mg/kgBB,
fraksi-fraksi pada dosis 140 mg/kgBB dan 280 mg/kgBB). Pengujian aktivitas
antidiabetes in vitro dari ekstrak dan fraksi dengan dengan metode inhibisi enzim
α amilase, inhibisi enzim α glukosidase.
Pada uji toleransi glukosa oral didapatkan hasil bahwa ekstrak etanol total 140
mg/kgBB, ekstrak SFE 140 mg/kgBB dan fraksi etil asetat ekstrak etanol residu
SFE 140 mg/kgBB secara signifikan mampu menahan pembebanan glukosa yang
diberikan pada hewan uji selama 120 menit pengamatan.
Pada hewan diabetes yang diinduksi aloksan didapatkan hasil penurunan kadar
glukosa darah secara signifikan pada kelompok ekstrak etanol total 140 mg/kgBB
dan kelompok fraksi etil asetat ekstrak residu SFE 140 mg/kgBB dan 140
mg/kgBB. Sedangkan dari hasil histologi organ pankreas hanya kelompok ekstrak
etanol total yang dapat memperbaiki kerusakan pulau langerhans ditandai dengan
lebih banyaknya jumlah pulau langerhans dan jumlah sel β pankreas dibandingkan
dengan kelompok diabetes.
Pada model hewan resisten insulin yang diinduksi dengan makanan tinggi lemak
dan karbohidrat, penurunan kadar glukosa terdapat pada kelompok ekstrak etanol
total 280 mg/kgBB, ekstrak SFE 140 mg/kgBB dan fraksi etil asetat ekstrak etanol
residu SFE 280 mg/kgBB. Kenaikan nilai konstanta tes toleransi insulin (KTTI)
terlihat pada kelompok ekstrak etanol total 280 mg/kgBB, ekstrak SFE 140
mg/kgBB dan fraksi etil asetat ekstrak etanol residu SFE 140 mg/kgBB. Hasil
histologi organ hati pada kelompok ekstrak SFE dan fraksi etil asetat ekstrak
etanol residu SFE menunjukkan adanya penurunan steaosis hepatis.
Pada hasil pengujian secara in vitro dengan uji α amilase dan α glukosidase,
menunjukkan tidak adanya aktivitas antidiabetes yang signifikan baik pada
ekstrak maupun fraksi buah krangean.
Hasil pengujian in vivo membuktikan bahwa ekstrak etanol total dosis 140
mg/kgBB, ekstrak SFE 140 mg/kgBB dan fraksi etil asetat dosis 140 mg/kgBB
memiliki aktivitas antidiabetes. Mekanisme kerja dominan dari ekstrak dan fraksi
etil asetat sebagai antidiabetes adalah dengan meningkatkan sensitifitas jaringan
terhadap insulin.

Diabetes mellitus (DM) is a group of metabolic disorders caused when the
pancreas does not produce insulin or the body cannot use insulin which is
produced effectively. Insulin is a hormone that regulates the balance of blood
sugar in the body so there will be no increase in glucose level in the blood.
The increase in number of people with DM continues to grow throughout the
world. Based on WHO data, Indonesia was ranked as the second largest in
SEARO (Southern Asian Pacific) countries with a percentage of deaths from
diabetes of 6%. The national prevalence of DM in 2018, based on the PERKENI
2015 consensus, increased to 10.9% from 8.5% according to PERKENI 2011
consensus.
At this time, DM is treated with synthetic drugs which have relatively high side
effects and more expensive price. In addition to conventional medicines, currently,
there is also an increase in people's interest in traditional medicines due to its
promising effectiveness, fewer side effects and cheaper medical costs.
Litsea cubeba (Lour.) Pers. or krangean is a plant which is widely found in the
highlands. The oil component of krangean is widely used for a treatment.
Krangean fruit itself has been empirically used as a treatment for diabetes by
using one raw krangean fruit which is chewed twice a day for 4-6 weeks.
However, further observation is still needed to find out what group of compounds
is responsible for the antidiabetic activity. Furthermore, how the mechanism of
these compounds is in reducing blood glucose level also needs to be investigated.
This research aims to find out the activity and to study the mechanism of the
krangean plant extracted by using Supercritic fluid extraction (SFE) with ethanol
solvents and fractionated by using n-hexane, ethyl acetate and water solvents. The
use of SFE itself aims to separate the components of oil with other chemical
compounds contained in the krangean fruit. To achieve this goal, the research
series was designed as follows: total extraction of krangean fruit using ethanol
solvent, extraction using SFE, extraction of SFE residues using ethanol solvent,
fractionation of krangean fruit’s extract residues using n-hexane and ethyl acetate

solvents. The testing of in vivo antidiabetic activity from extracts and fractions
was carried out by using oral glucose tolerance test method, alloxan induction
method and insulin resistance induction method with high-fat-carbohydrate foods
in test animals (all in vivo tests were carried out with multilevel doses namely a
dose of 140 mg/kg , 280 mg/kgBB and 560 mg/kgBB for the extract, and a dose of
140 mg/kgBB and 280 mg/kgBB for the fractions). The testing of in vitro
antidiabetic activity of extracts and fractions was conducted by using α amylase
and α glucosidase enzyme inhibition methods .
The oral glucose tolerance test showed that the total ethanol extract was 140
mg/kgBB, SFE extract was 140 mg/kgBB and the ethyl acetate fraction of SFE
residual ethanol extract was 140 mg/kgBB. It was significantly able to withstand
the loading of glucose given to the test animals for 120 minutes of observation.
In alloxan-induced diabetic animals, it was found that blood glucose levels
decreased significantly in the total ethanol extract group of 140 mg/kgBB and the
ethyl acetate fraction of SFE residual extract of 140 mg/kgBB and 140 mg/kgBB.
Whereas from the result of histology of the pancreatic organs, it was only the total
ethanol extract group that could repair the damage to the islets of Langerhans
characterized by increasing number of islets of Langerhans and the pancreatic β
cells compared to the diabetic group.
In insulin-resistant animals model induced with foods that are high in fat and
carbohydrates, a decrease in glucose levels was found in the total ethanol extract
group of 280 mg/kgBB, SFE extract of 140 mg/kgBB and the ethyl acetate fraction
of SFE residual ethanol extract of 280 mg/kgBB. The increase of constants value
in insulin tolerance test (KTTI) was seen in the total ethanol extract group of 280
mg/kgBB, SFE extract of 140 mg/kgBB and the ethyl acetate fraction of SFE
residual ethanol extract of 140 mg/kgBB. The results of liver histology in the SFE
extract group and ethyl acetate fraction of SFE residual ethanol extract showed a
decrease in hepatic steatosis.
The results of in vitro test with α amylase and α glucosidase tests showed no
significant antidiabetic activity in both the extract and fraction of the krangean
fruit.
The results of in vivo test proved that the total ethanol extract dose of 140
mg/kgBB, SFE extract of 140 mg/kgBB and the ethyl acetate fraction dose of 140
mg/kgBB have antidiabetic activity. The dominant mechanism of action of the
extract and ethyl acetate fraction as antidiabetic is by increasing tissue sensitivity
towards insulin.
