    HIV INFECTION IN HB E THALASSEMIA CHILD

    Abstrak :

    Background:Children with thalassemia major survive mainly on regular blood transfusions, which may

    expose them to different transfusion-transmitted infections (TTIs), especially hepatitis B, hepatitis C, HIV,

    and Treponema pallidum. The risk of HIV transmission is estimated to be 1:1.930.000, although it isdifficult to be precise with such a low incidence.

    Objective:To report a case of HIV infection in a child with Hb E Thalassemia

    Case Report:R, 14 year-old girl with Hb E Thalassemia, has received regular blood transfusion in

    Tulungagung and Surabaya

    hospital since she was 3 years old. She was planned to undergo splenectomy because of the massive

    enlargement of spleen which causes concern about possible splenic rupture. Preoperation laboratory

    examination for HIV infection using ELISA method was positive but CD4 absolut was normal and Hbs Ag

    or anti HCV were negative. The ELISA method for HIV infection examination for her parents was negative.

    There were no history of drug abuse, sexual intercourse, reuse of contaminated needles and syringe, and

    tattoing/traditional piercing. The antiviral drug has not been given because CD4 absolute level was more

    than 500 cell/uL and the patient was in a good condition.Conclusion:A

    case of HIV infection in a child with HbE Thalassemia, we assumed HIV infection was acquired through

    regular transfusion.
