Membedakan Sesak Nafas Akibat Gangguan Kardiovaskular dan Bukan
Dyspnea mengacu pada sensasi sulit bernapas atau tidak nyaman dalam bernafas. Hal
tersebut merupakan pengalaman subyektif yang dirasakan dan dilaporkan oleh pasien
yang terkena. Dyspnea harus dibedakan dari takipnea, hiperventilasi, dan hiperpnea, yang
merujuk pada variasi pernapasan terlepas dari sensasi subyektif pasien. Takipnea adalah
peningkatan laju pernafasan di atas normal; hiperventilasi merupakan peningkatan
ventilasi relatif terhadap kebutuhan metabolisme, dan hiperpnea adalah peningkatan yang
tidak seimbang dalam ventilasi relatif terhadap peningkatan tingkat metabolisme. Kondisi
ini mungkin tidak selalu berkaitan dengan dyspnea. 1
Dua jenis yang tidak umum dari sesak napas yang trepopnea dan platypnea. Trepopnea
adalah dyspnea yang terjadi dalam satu posisi dekubitus lateral dibandingkan dengan yang
lain. Platypnea mengacu pada sesak napas yang terjadi dalam posisi tegak dan lega pada
posis berbaring.
Dyspnea on exertion tidak berarti selalu indikasi penyakit. Orang normal mungkin merasa
dyspneic dengan latihan berat. Tingkat aktivitas ditoleransi oleh setiap individu tergantung
pada variabel seperti umur, jenis kelamin, berat badan, kondisi fisik, sikap, dan motivasi
emosional. Dyspnea on exertion dianggap indikasi penyakit bila terjadi pada tingkat
aktivitas yang semestinya dapat ditoleransi dengan baik. Kita perlu menanyakan apakah ia
telah melihat keterbatasan baru yang progresif mengenai kemampuannya untuk
melakukan tugas-tugas tertentu yang ia mampu lakukan tanpa kesulitan di masa lalu
(misalnya, berjalan, naik tangga, melakukan pekerjaan rumah tangga). Tingkat gangguan
fungsional dapat dinilai dengan cara ini.
Seorang pasien dengan dispnea mungkin berkata, “Saya merasa: sesak napas, mengalami
kesulitan bernapas, tidak bisa bernapas, seperti tercekik”. Karena merupakan fenomena
subyektif, persepsi dan interpretasi dyspnea bervariasi dari pasien ke pasien. Kita dapat
memulai dengan sebuah pertanyaan terbuka berupa, “Apakah Anda memiliki kesulitan
bernapas?” Jika respon berupa afirmatif dan dyspnea dianggap menjadi masalah, kita perlu
mengetahui detail kapan onsetnya terjadi, apakah onset tiba-tiba atau bertahap, faktor
pemberat dan yang memperingan serta frekuensi dan durasi serangan. Kondisi-kondisi di
mana dispnea terjadi harus dipastikan di antaranya adalah respon terhadap aktivitas,
kondisi emosional, dan perubahan posisi tubuh harus diperhatikan Tanyakan juga tentang
gejala-gejala yang terkait seperti nyeri dada, palpitasi, mengi, atau batuk Kadang-kadang
batuk tidak produktif dapat muncul setara dengan dyspnea. Riwayat pasien juga harus
diketahui mengenai masalah medis signifikan yang dimiliki pasien, obat apa yang dia telah
minum serta berapa banyak dia merokok? Juga mengenai riwayat asma, gangguan paru,
alergi dan demam tinggi perlu dipertimbangkan. 1, 2
Pertanyaan tambahan harus ditujukan untuk memastikan apakah pasien memiliki
ortopnea atau dispnea nokturnal paroksismal. Menanyakan tentang jumlah bantal ia yang
ia gunakan di bawah kepalanya pada malam hari dan apakah ia pernah harus tidur dengan
posisi duduk. Apakah dia mengalami batuk atau mengi pada posisi berbaring? Apakah ia
pernah terbangun di malam hari dengan sesak napas? Berapa lama setelah berbaring
melakukan episode terjadi, dan apa yang dia lakukan untuk meringankan penderitaannya
tersebut? 1, 3
Pada kejadian dispnea, kita bisa mengklasifikasikan penyebabnya menjadi 4 kategori
utama, yaitu kardiak, pulmonari, campuran kardiak dan pulmonari serta bukan keduanya.
Radiografi dada, elektrokardiograf dan skrining spirometry dapat memberikan informasi
yang berharga untuk memastikannya. Pada kasus yang belum dapat dipastikan serta
membutuhkan klarifikasi, tes fungsi paru, pengukuran gas darah arteri, ekokardiograf dan
tes standard exercise treadmill atau tes complete cardiopulmonary exercise dapat
dilakukan.2
Sesak nafas atau dyspnea biasanya merupakan keluhan paling awal dan signifikan pada
pasien dengan keluhan gagal jantung kiri. Juga, seringkali disertai dengan batuk karena ada
transudat cairan ke dalam rongga udara. Kerusakan yang lebih lanjut dapat menyebabkan
pasien mengalami dyspnea saat berbaring yang juga disebut orthopnea. Hal tersebut dapat
terjadi karena terjadi peningkatan pengembalian darah vena dari ekstremitas bawah dan
elevasi diafragma saat berada dalam posisi supinasi. Karena itu juga, pasien akan merasa
lebih baik saat duduk maupun berdiri atau dengan mengganjal bagian atas tubuh dengan
bantal yang tinggi sehingga rongga dada cenderung naik ke atas. Pasien dapat pula
mengalami paroxymal nocturnal dyspnea, berupa tiba-tiba terbangun saat sedang tidur
karena tidak bisa bernafas. 4, 5
Pada gagal ventrikel kiri awal, output jantung tidak meningkat dengan cukup sebagai
respon terhadap olahraga ringan sedang sehingga asidosis jaringan dan otak terjadi, dan
pasien mengalami dyspnea on exertion. Sesak napas dapat disertai dengan kelelahan atau
sensasi mencekik atau kompresi sternum. Pada tahap selanjutnya dari kegagalan ventrikel
kiri, sirkulasi paru-paru tetap mengalami kongesti, dan dispnea dapat terjadi dengan
tenaga yang lebih ringan.
Selain itu, pasien dapat mengalami ortopnea atau paroxymal nocturnal dyspnea. Edema
paru akut adalah manifestasi paling dramatis dari kelebihan overload vena paru-paru dan
dapat terjadi pada infark miokard baru atau pada tahap terakhir dari kegagalan ventrikel
kiri kronis. Kardiovaskular penyebab dispnea di antaranya adalah penyakit katup (stenosis
mitral dan insufisiensi terutama aorta), arrhythmia paroksismal (seperti atrial fibrilasi),
efusi perikardial dengan tamponade, hipertensi sistemik atau paru-paru, kardiomiopati,
dan miokarditis.Asupan atau administrasi cairan pada pasien dengan gagal ginjal oliguri
juga kemungkinan dapat berperan pada terjadinya kongesti paru dan dyspnea.
Sementara itu, penyakit paru yang merupakan kategori utama lain penyebab terjadinya
dyspnea , di antaranya adalah asma bronkial, penyakit paru obstruktif kronik, emboli paru,
pneumonia, efusi pleura, pneumotoraks, pneumonitis alergi, dan fibrosis interstisial. Selain
itu, dyspnea mungkin terjadi pada demam dan kondisi hipoksia serta berhubungan dengan
beberapa kondisi kejiwaan seperti kecemasan dan gangguan panik. Diabetic ketoacidosis
jarang menyebabkan dypsnea namun pada umumnya menyebabkan pernafasan lambat
dan dalam (pernafasan Kussmaul. Lesi serebral atau perdarahan intrakranial mungkin
terkait dengan hiperventilasi kuat dan kadang-kadang napas tidak teratur periodik disebut
pernafasan Biot. Hipoperfusi cerebral dari sebab apapun juga dapat mengakibatkan
periode hiperventilasi dan apnea disebut respirasi Cheyne-Stokes, meskipun mungkin
tidak ada kesulitan bernapas dirasakan oleh pasien.
Pada emfisema, sesak nafas juga merupakan tanda pertama dari gejalanya. Emfisema
merupakan penyakit sumbatan jalan nafas kronik yang ditandai dengan pembesaran
permanen pada jalan nafas bagian distal ke terminal bronkiolus. Awalnya tampak diam-
diam tetapi progresif. Pada pasien yang memang memiliki bronkitis atau asma bronkitis
kronik, batuk dan mengi mungkin menjadi penanda awal. Gambaran klasik pada pasien
yang tidak memiliki komponen bronkitis adalah mengalami barrel-chest dan dispnea
dengan expirasi yang lebih lama, duduk ke depan pada posisi membungkuk,berusaha
menekan udara keluar paru-paru dengan expiratory effort. Pada pasien tersebut, rongga
udaranya membesar dan kapasitas difusinya turun. Dispnea dan hiperventilasi sangat
mencolok sehingga sampai penyakit tahap akhir, pertukaran gas masih adekuat dan nilai
gas darah masih relatif normal.
Pasien emfisema lain yang ekstrem serta memiliki bronkitis kronik dan riwayat infeksi
berulang dengan sputum purulen biasanya memiliki dyspnea yang kurang mencolok serta
dorongan nafas. Hal tersebut menyebabkan mereka akan menahan karbon dioksida
sehingga hipoksia dan seringkali sianosis. 4
Untuk bisa mengerucutkan pada suatu diagnosis penyebab sesak nafas, perlu dilakukan
pemeriksaan fisik lengkap sehingga tidak perlu melakukan pemeriksaan laboratorium.
Patologi orofaringeal atau nasofaring dapat ditemukan dengan mengidentifikasi kelainan
obstruktif kasar dari bagian hidung atau tenggorokan. Palpasi leher dapat mengungkapkan
massa, seperti di thyromegaly, yang dapat berkontribusi untuk obstruksi saluran napas.
Bruits leher adalah indikasi penyakit makrovaskuler dan mengarahkan pada penyakit
arteri koroner, terutama jika pasien memiliki riwayat diabetes, hipertensi atau merokok.
Pemeriksaan thorax dapat menunjukan peningkatan diameter anteroposterior, tingkat
pernapasan tinggi, kelainan bentuk tulang belakang seperti kyphosis atau scoliosis, bukti
trauma dan penggunaan otot aksesori untuk bernapas. Kyphosis dan scoliosis bisa
menyebabkan pembatasan paru. Auskultasi paru-paru memberikan informasi mengenai
karakter dan simetri nafas suara seperti rales, ronki, suara tumpul atau mengi. Rales atau
mengi dapat mengindikasikan gagal jantung kongestif, dan ekspirasi mengi saja dapat
mengindikasikan penyakit paru-paru obstruktif.
Pemeriksaan kardiovaskular dapat menunjukan murmur, suara jantung tambahan,
kelainan dari detak atau irama jantung. Sebuah murmur sistolik dapat menunjukkan
stenosis aorta atau insufisiensi mitral, sebuah suara jantung ketiga dapat mengindikasikan
gagal jantung kongestif dan ritme yang tidak teratur bisa menunjukkan fibrilasi atrial.
Perfusi perifer ekstremitas harus dievaluasi dengan menilai pulsasinya, kapillari refill,
edema dan pola pertumbuhan rambut.
Pemeriksaan psikiatrik dapat mengungkapkan kecemasan disertai dengan gemetar,
berkeringat atau hyperventilation.2

Paroxysmal Nocturnal Dyspnea
Imagine awakening suddenly from sleep, gasping for air, coughing, wheezing and feeling
like you're suffocating. Anxious and scared, you might quickly sit up and put your feet on
the floor. Sitting or standing up helps you breathe better again. This frightening occurrence
is a sign of heart failure -- and it sometimes happens to people who haven't had any
previous symptoms of heart trouble.
The scientific term for this occurrence is paroxysmal nocturnal dyspnea. "Paroxysmal"
means "sudden attacks that recur," "nocturnal" means "occurring at night" and "dyspnea"
means "difficulty breathing" or "shortness of breath."
Paroxysmal nocturnal dyspnea is also sometimes called cardiac asthma because its signs
and symptoms mimic those of asthma. This term is actually misleading -- the two
conditions are very different. While asthma is a chronic condition caused by inflammation
of the airways, which can lead to breathing difficulties, paroxysmal nocturnal dyspnea is a
sign of heart failure.
Paroxysmal nocturnal dyspnea can be accompanied by swelling of the feet or ankles. It's
caused by congestion (excessive or abnormal accumulation of blood) in the lungs, perhaps
along with accumulation of excess fluid in the lungs (pulmonary edema), which occurs as
a result of left-sided heart failure.
Some patients who experience paroxysmal nocturnal dyspnea also experience orthopnea:
breathlessness when lying down that is relieved when several pillows raise the head and
upper body. People with this condition must keep their heads elevated to breathe
comfortably. Paroxysmal nocturnal dyspnea is an intense form of orthopnea. It commonly
occurs several hours into sleep (as opposed to immediately upon lying down), and it's
relieved by sitting upright, but not as quickly as is orthopnea.
Next, we'll learn what causes paroxysmal nocturnal dyspnea and how you can treat it.
Causes of Paroxysmal Nocturnal Dyspnea
Paroxysmal nocturnal dyspnea and orthopnea have similar causes. One of the causes of
orthopnea is pulmonary congestion (excessive or abnormal accumulation of blood in the
lungs) when a patient lies down and blood moves from the lower part of the body to the
lungs. In normal individuals this has little effect, but in heart failure patients, the diseased
left side of the heart can't pump out the additional volume. So blood backs up in the vessels
of the lungs, causing shortness of breath.
Also, lung circulation in patients with congestive heart failure may already be overloaded.
This condition may be worsened by fluid overload, excess fluid that accumulates in the
lower part of the body during the day and moves into the lungs when the patient lies down.
Lung congestion decreases when the patient raises his upper body.
In paroxysmal nocturnal dyspnea, increasing amounts of fluid enter the lungs during sleep
and fill the small, air-filled sacs in the lungs where oxygen is absorbed from the
atmosphere. The failing left side of the heart is suddenly unable to match the output of a
more normally functioning right side, resulting in congestion in the lungs, coughing and
wheezing, and sometimes the production of sputum (mucus and other matter, which may
be bloody). We don't know why paroxysmal nocturnal dyspnea occurs so suddenly in
patients with left heart failure.
How is paroxysmal nocturnal dyspnea treated?
Treatment depends upon the exact cause of the episodes. Doctors generally recommend
weight reduction for obese patients, and supplemental oxygen may ease symptoms. If the
episodes are due to heart failure, steps are taken to treat fluid overload.
Because a diet high in salt can contribute to fluid overload, doctors generally recommend
that people who experience breathing difficulties such as paroxysmal nocturnal dyspnea
limit the amount of salt in their diets. In addition to not adding salt at the table, following a
low-sodium diet involves avoiding most processed foods (including frozen and canned
foods, as well as the majority of fast food and takeout food items). Eating home-cooked
meals prepared with herbs and spices instead of salt is one of the most effective ways of
getting rid of excess fluid in the body.
In addition to recommending a low-sodium diet, doctors often prescribe medicines known
as diuretics ("water pills") to relieve the symptoms of fluid overload. While these methods
work for a while, they often become ineffective over time. When that happens, people with
fluid overload are typically admitted to the hospital, where they're treated with
intravenous medicines like loop diuretics, vasodilator drugs and inotropic drugs. Another
procedure, known as ultrafiltration, is available to relieve fluid overload. In ultrafiltration
therapy, the person's blood is passed through a filter that removes excess fluid. Then the
filtered blood (minus the excess fluid) is returned to the patient.

Orthopnea Overview
Counting your pillows, not sheep, as you go to sleep at night might reveal a telltale sign of a
serious heart condition.
People with some types of heart or lung problems often complain about difficulty breathing
while lying down. For many with this condition, this trouble breathing or sensation of
breathlessness is relieved only by sitting up straight or standing. Sometimes a person
realizes that sleep is more comfortable when propped up with extra pillows under his or
her head. This condition is called orthopnea, and it's commonly measured by the number
of pillows needed to ease the breathing ("three-pillow orthopnea, for example").
The word orthopnea comes from the Greek terms "orthos," which means "standing upright
or straight," and "pnoe," which means "breathing."
It's important to report orthopnea to your doctor, even if you can control it by simply
propping your head up while you sleep -- it may be a sign of heart failure danger. One study
has shown that heart failure patients with persistent orthopnea have poorer prognoses and
are at significantly higher risk of hospitalization than patients without orthopnea. The
study concluded that this group of patients requires a more aggressive approach to
improve their outcome.
What causes orthopnea?
In general, shortness of breath (dyspnea) is a common symptom of congestive heart
failure. This is because increased fluid in the lungs can interfere with the exchange of
oxygen, resulting in not enough oxygen reaching the tissues, This is most typically noticed
during exercise, but it can also occur when a person is lying down -- that's when it's called
orthopnea.
Orthopnea can be a subtle condition, in which a person notices that sleep is more
comfortable with several pillows under his head, or a more severe one in which he can
breathe deeply and comfortably only when sitting or standing. It may occur along with
swelling of the feet or ankles. Orthopnea is a sign of heart failure. It is caused by congestion
in the lungs -- and perhaps accompanied by accumulation of excess fluid in the lungs
(pulmonary edema) -- that occurs as a result of left-sided heart failure.
Orthopnea is caused by the accumulation of too much fluid in the lungs (pulmonary
congestion or fluid overload) when a person lies down. In a prone position, blood volume
from the feet and legs redistributes to the lungs, which doesn't cause a problem for people
whose hearts are pumping normally. However, in people with heart failure, excess blood in
the lungs can cause breathing problems or shortness of breath.
Fluid Overload Treatment
If excess fluid is causing breathing problems like orthopnea or paroxysmal nocturnal
dyspnea, doctors may recommend that the patient eat a low-sodium diet, because too much
sodium can contribute to fluid overload. They may also prescribe diuretics (sometimes
called "water pills"), medicines that help the body eliminate excess sodium and water.
However, diuretics sometimes become less effective over time, so after awhile, many
people with fluid overload must be hospitalized for additional treatment.
This treatment can include intravenous medications like loop diuretics, vasodilator drugs
(medicines that increase the diameter of blood vessels so blood flow is increased) and
inotropic drugs (medicines that cause the heart to pump more blood with each
contraction).
If these medicines don't work to relieve the fluid overload, doctors may prescribe a
procedure known as ultrafiltration, in which blood is passed through a small machine that
filters out the excess fluid.

Paroxysmal Nocturnal Dyspnea
Paroxysmal nocturnal dyspnea, a condition related to orthopnea, is experienced by some
people with heart failure. It's a frightening attack of shortness of breath that can awaken a
person suddenly from sleep and that is relieved only by sitting or standing up. Paroxysmal
nocturnal dyspnea isn't relieved as quickly as is orthopnea -- and it doesn't occur right after
the person lies down, but usually one or two hours after they fall asleep. People with heart
failure may experience orthopnea, paroxysmal noctural dyspnea, both, or neither.
