Respons imun terhadap bakteriDefinisi, struktur dan klasifikasiBakteri merupakan mikroba prokariotik uniselular, berkembang biak secara aseksual dengan pembelahan sel.KLASIFIKASI BAKTERIMorfologi/bentuk bakteriBerdasarkan berntuknya, bakteri dibagi menjadi tiga golongan besar, yaitu: Kokus (Coccus) dalah bakteri yang berbentuk bulat seperti bola, dan mempunyai beberapa variasi sebagai berikut:o Mikrococcus, jika kecil dan tunggalo Diplococcus, jka bergandanya dua-duao Tetracoccus, jika bergandengan empat dan membentuk bujursangkaro Sarcina, jika bergerombol membentuk kubuso Staphylococcus, jika bergerombolo Streptococcus, jika bergandengan membentuk rantai

(Dikutip dari : Oberholzer A, Oberholzer C, Moldawer LL. Cytokine signalingregulation of the immune response in normal and critically ill states Crit Care Med 2000(4);28: 3-12)

Tanda radangMakroskopikrubor (merah)tumor (bengkak)kalor (panas)dolor (nyeri)functio laesa

MikroskopikVasodilatasiEksudasiEmigrasi leukositMekanisme radang akut

Jejas ( trauma, zat kimia, mikroorganisme, dll) menyebabkan adanya kerusakan jaringan.Kerusakan jaringan merangsang munculnya mediator. Mediator adalah zat kimia yang dihasilkan oleh plasma atau sel, yang berfungsi menentukan berbagai reaksi radang.Permeabilitas pembuluh darah meningkat menyebabkan keluarnya beberapa protein plasma beserta cairan plasma (eksudasi).

Penyebab radangAgen infektif:Bakteri/kuman/basilGolongan coccusGolongan virusGolongan RicketsiaGolongan ClamidiaGolongan MikobakteriumGolongan ParasitGolongan jamur-jamurVaskular/hormon

LO 3. Patofisiologi infeksi bakteriThe infectious processentryColonization, adhesion and invasionPathogenic actionPortal of entryKulitMembran mukosaRespiratory, gastrointestinal, urinary, reproductive tractsplasentaAdhesion / AttachmentAdhesion factors: Specialized structuresAdhesion disk (protozoa), suckers, hooks(helminthes) Ligands: Surface lipoprotein andglycoprotein (bacteria, viruses) Adhesin (bacteria): found on fimbrae, flagella, glycocalyses Attachment protein (viruses)

N.gonorrhoeae : has adhesin on its fimbrae that adhere to cells lining the urethra and vagina of humanSome bacterial pathogens do not attach to hostcells directly, but instead interact with each other to form a sticky web of bacteria and Polysaccharides called a biofilm, which adheres to a surface within a host (example: dental plaque)

Pathogenicity : The ability a of microorganism to cause diseaseVirulence : Degree of pathogenicityVirulence factorsA variety of traits that interact with a host and enable the pathogen to enter a host, adhere to host cells, gain access to nutrients, and escape detection or removal by the immune system

Virulence FactorsExtracellular Enzymes Hyaluronidase and collagenase :degrade specific molecules to enable bacteria to invade deeper tissue Coagulase :blood protein, providing a hiding place for bacteria within a clot Kinase :such as staphylokinase and streptokinase digest blood clots

Toxins ExotoxinCytotoxins : kills host cells in general or affect their functionNeurotoxins : specifically interfere with nerve cell functionEnterotoxins : affects cell lining the gastrointestinal tract EndotoxinLipid A, lipid portion of the membranes lypopolysaccharide

Antiphagocytic factors CapsulesComposed of chemicals that are normally found in the body (including Polysaccharides) do not stimulate a hosts immune response Antiphagocytic chemicals Prevent the fusion of lysosomes with phagocytic vesicles, which allows the bacteria to survive inside of phagocytes

The symptoms of a bacterial infection can be produced by excessive immune and inflammatory responses triggered by the infectionContoh:Endotoxin : acute phase protein can cause the life-threatening symptoms associated with sepsis and meningitisTypes of Infection and Their Role in Transmission
