D I F T E R I
• penyakit infeksi toksik akut, menular disebabkan
Corynebacterium diphtheriae ditandai pembentukan
pseudomembran pada kulit dan/atau mukosa
PATOGENESIS • C diphtheriae masuk melalui hidung & mulut (tipe
GRAVIS, MITIS, INTERMEDIUS, BELFANTI)• Basil tetap pada permukaan mukosa saluran nafas,
kadang mukosa mata/genitalia• Setelah masa tunas 2 - 4 hari strain lysogenized
menghasilkan toksin• Bakteri membuat toxin (racun) bila terinfeksi oleh
virus (pembawa tox gen)
GEJALA KLINIS Bervariasi dari tanpa gejala
fatal• Lesi khas sebagai suatu membran asimetrik (tak simetri)
keabu-abuan dikelilingi oleh daerah inflamasi• Tenggorokan sakit (Nyeri telan), Kelenjar limfe membesar &
melunak. Penyumbatan jalan nafas / sesak nafas• Adanya oedema & pembengkakan di leher pd kasus sedang &
berat (BULLNECK) Keluhan dan gejala tergantung : tempat infeksi status imunitas penjamu distribusi toksin kedalam sirkulasi
PENULARAN• Penyakit “ re emerging “ (meningkat kembali)
• Masa Penularan beragam, tetap menular sampai hilangnya bakteri di lesi. Biasanya berlangsung 2 minggu atau kurang.
• Carrier kronis ( pengidap tapi tak sakit ) dapat menularkan penyakit sampai 6 bulan

DEFINISI OPERASIONAL (1)
Kasus Suspek• adalah orang dengan gejala
Laringitis, Nasofaringitis atau Tonsilitis ditambah pseudomembrane putih keabuan yang tak mudah lepas dan mudah berdarah di faring, laring, tonsil.
DEFINISI OPERASIONAL (2)
Kasus Probable : adalah orang dengan suspek difteri ditambah salah satu dari : Pernah kontak dengan kasus (<2 minggu) Ada didaerah endemis difteria Stridor , Bullneck Pendarahan Submucusa atau petechiae pada kulit Gagal jantung toxic, Gagal ginjal akut Myocarditis and/or kelumpuhan motorik 1 s/d 6 minggu setelah onset Mati
Kasus konfirmasi• orang kasus probable yang hasil isolasi ternyata
positiv C difteriae yang toxigenic (dari usap hidung, tenggorok, ulcus kulit, jaringan,
conjunctiva, telinga, vagina) • atau
• serum antitoxin meningkat 4 kali lipat atau lebih (hanya bila kedua sampel serum diperoleh
sebelum pemberian toxoid difteri atau antitoxin)

Apa yang dilakukan saat klb
• Menekan kematian kasus (short term)– Deteksi dini dan rujukan – Manajemen kasus
• Menekan transmisi & kasus baru (short term)– PE– ORI di daerah kasus
• Mencegah KLB (long term)– Meningkatkan cakupan – Meratakan cakupan, meniadakan kantong
mengapa ada yang tidak kebal
• Sebelum vaccine era transmisi kuman sangat kuat, sehingga dapat menimbulkan kekebalan alamiah
• Transmisi berkurang akibat– Perbaikan sanitasi dan lingkungan hidup– kekebalan manusia akibat vaccine
• Kegagalan imunisasi – Tidak imunisasi– Gagal imunisasi– Mempertahankan UCI
–Cakupan kurang tinggi
M A S A L A H (1) • Profilaksis tidak optimal, 1) Hanya sebagian kecil kontak yg kena profilaksis 2) Pemantauan minum obat sulit 3) Efek samping obat
4) Kemungkinan DO besar
• Kasus masih tinggi 1) sosialisasi aktif kasus meningkat
2) intervensi terbatas tidak optimal 3) kerier sudah menyebar dimana-mana 4) profilaksis tidak optimal 5) Masih muncul kasus baru di wil. Non ORI wil.ORI kurang luas 6) Masih muncul kasus baru di wil. ORI status “D“ MASIH < 3X
Imunisasi difteri
• Maternal antibodi sampai 4 bulan• Bayi mulai usia 6-8 minggu
– dengan vaksin DTP, DTaP– Interval minimal 4 minggu
• Booster pertama pada usia 18 bulan, 3 tahun atau 4 tahun dengan vaksin DTP
• Booster kedua pada usia sekolah dengan Td• Setiap 10 tahun dengan Td
upaya penguatan imunisasi
Alternatif :• Penguatan Imunisasi rutin + Imunisasi tambahan
minimal 1x (massal) seperti PIN s/d 15 thn • Penguatan Imunisasi rutin secara keseluruhan pada
bayi kohort + imunisasi tambahan pada semua usia • Penguatan imunisasi + Imunisasi tambahan terbatas
populasi at risk menurut umur / survei serologik• Penguatan imunisasi rutin + imunisasi tambahan
pada daerah kantong saja • Catch up immunization di daerah kantong saja
