1. Syarat penulisan resep:

Tanggal penulisan resep Nama dokter Nama pasien

2. Setiap permulaan resep atau obat baru harus di tulis R/

3. Setiap penulisan obat harus disertai kemasan obat:

Capsul Vial Tablet : tab Ampul : amp

4. Penulisan jumlah obat harus menggunakan huruf Romawi

Contoh : R/ Fepiram 3g amp. No IV(empat) ∫ pro injeksi

Contoh: R/ Neulin PS caps No X (10) ∫ 2dd I

Contoh : R/ NaCl 0,9% 100 cc fl. No II ∫ pro infus

5. Penulisan resep alat medis menggunakan i.m.m setelah ∫

Contoh: R/ Catheter No. 16 No I ∫ i.m.m

Contoh: R/ Nasogastric Tube No. 8 No I ∫ i.m.m

6. Penulisan resep disposable bukan spuit

Contoh: R/ Disposable 20 cc No IV ∫ pro injeksi R/ Needle No. 23 No IV ∫ pro injeksi
