csContraindications for use of Blood

Glucose MeterDue to the Hazard Notice the MHRA recommend that therapeutic decisions should not be based on a capillary sample in these circumstances:

LIMITATIONS.

Due to medical conditions:

• Galactose > 1.11 mmol/L (overestimation)

• Lipids > 16.8 mmol/L (overestimation)

• Sodium > 150 mmol/L (underestimation)

• Check expiry dates, mix QC solutions and repeat test

• Change quality control solutions (write new 3 month expiry date)

• Document all findings in QC log book

Verification of Blood Sample

• Always perform a visual inspection of test area.

• If yellow area is visible – Perform a repeat test ensuring yellow area fully covered.

Important Error CodesMeter Reading Range 0.6mmol/l – 33.3mmol/l

Send a venous sample for verification if reading < 3mmol/l or > 20mmol/l or according to your Trust Policy/Guidelines for Blood Glucose Monitoring.

Please refer to Ward Manual/Logbook for all display/error codes. Error codes which must be acted upon immediately are shown below, these codes indicate the result is potentially outside the meter

reading range:

Potentially very low glucose

Potentially very high glucose

ERROR CODES: (see back of log book)

• The temperature is above or below the proper range - i.e 6 – 44oC. Meter requires to be warmed & takes 10mins before usable.

• The code chip is from expired lot of test strips. Insert new in date code chip.
