APA ITU INVESTIGASI KECELAKAAN KERJA

Merupakan suatu upaya yang dilakukan secara sistematis dalam rangka :

Mengidentifikasi faktor-faktor penyebab kecelakaan Menerapkan tindakan penanggulangan & pencebagahan

kecelakaan Merupakan salah satu upaya inti dalam pelaksanaan

manajemen K3.Sistematis

Menggunakan pendekatan/metode Suatu tahapan proses (input proses output : mulai

dari sumber daya yang dibutuhkan, bagaimana menerapkannya, bagaimana mengevaluasi hasil ivestigasi sehingga dapat digunakan dalam pengembangan kebijakan K3 di waktu datang

TUJUAN INVESTIGASI KECELAKAAN DI TEMPAT KERJA

Untuk memberikan dan meningkatkan upaya yang lebih sistematis dapat memberikan manfaat terhadap :

Identifikasi faktor-faktor penyebab kecelakaan

Memberikan rekomendasi untuk tindakan perbaikan

Mencegah dan atau mengurangi risiko terjadinya kecelakaan di waktu mendatang

The accident analysis process must determine not only the specific unsafe acts and conditions that contributed to the accident, but also the deficiencies in the management system that allowed the accident to occur.

A systematic approach to accident investigation and analysis is essential to an effective injury and illness prevention program. A through analysis will:• Provide the information needed to identify trends and problem areas such as the

need for further training.• Identify, without placing blame, the primary and secondary causal factors that

contributed to each accident.• Identify deficiencies in the management system.• Suggest corrective actions to prevent the reoccurrence of a similar accident.• Provide information that will help determine the compensability of the claim.• Demonstrate management’s commitment to provide a safe and healthy work

environment.• Identify previously unrecognized unsafe practices or conditions.• Maintain employee awareness of the importance of safe work habits.

The Middle Managers Sometimes investigations require

participation by middle or higher level managers. Typical situations are when :

• There is major loss or high potential accident

• The circumstances cross into other supervisors

• The remedial action have a broad scope or significant costs

Staff personnel • Occasionally, special knowledge is needed in

an investigation. This could be because of a new process, suspected failure, use of hazardous materials, or a a complex situation.

• Part of the information may be need to be obtained or analyzed by a technical expert.

• Such expert then become advisors to the investigators

• Safety personnel are included in the group as technical staff advisors
