Saya adalah mahasiswa Program Studi Kesehatan Masyarakat peminatan Keselamatan dan
Kesehatan Kerja (K3) Universitas Esa Unggul yang sedang melakukan penelitian di wilayah
kerja PT. Pamindo Tiga T Tangerang. Penelitian ini bertujuan untuk mengetahui hubungan
perilaku kerja dengan kejadian kecelakaan kerja pada pekerja di Unit Produksi.
Saya mengharapkan partisipasi bapak/ibu dalam memberikan jawaban atas wawancara sesuai
dengan pendapat bapak/ibu tanpa dipengaruhi orang lain. Saya akan menjamin kerhasiaan
identitas dan jawaban bapak/ibu, jawaban yang diberikan hanya untuk kebutuhan penelitian.
