Nama : Angele Boudreaux Umur : 19 tahun

Keluhan pasien :

Angele Boudreaux mengeluh mengalami tersumbat di hidung, tidak dapat tidursemalaman, setiap bangun tidur mulutnya terasa kering dan tetap kering

sepanjang hari. Terkadang ia mengalami bersin dan tidak dapat berhenti. Saat iamencoba menghentikan bersinnya, hidungnya terasa kembali tersumbat. Iamerasa kesulitan di sekolah karena selalu merasa lelah dan sekarang matanyaterasa gatal dan berair setiap saat. Gejala ini telah terjadi sejak ia kecil danmemburuk pada musim gugur dan berkurang pada musim semi namun gejala inimuncul terus menerus selama 7 bulan terakhir. Namun gejala gatal dan berair

pada mata yang dialaminya saat ini tidak terjadi pada gejala rhinitis yang ia alamisebelumnya. Ia tidak mengalami demam dan sakit tenggorokan namun ia sesekalimengalami batuk tidak produktiv yang memburuk pada malam hari. Keluhan lainyang dialami oleh Angele adalah sakit kepala sesekali, mengi, nyeri dada danketidaknyamanan perut tapi tidak mengalami sesak nafas.

Riwayat penyakit :Angele Boudreaux mengalami alergi rhinitis sejak 14 tahun yanglalu.

Riwayat pengobatan :Angele Boudreaux pernah menjalani tonsilektomi danadenoidectomy pda usia 8 tahun, pernah menjalani rekontruksianterior ligament pada usia 16 tahun. Angele menderita alergi

terhadap kodein karena menyebabkan gatal-gatal.

Riwayat keluarga :Ayahnya berusia 43 tahun dengan sejarah penyakit HTN dan hiperlipidemia.Ibunya berusia 39 tahun dengan sejarah penyakit depresi. Adik laki-lakinya yangberusia 17 tahun menderita asma persisten sedang sedangkan adikperempuannya menderita alergi rhinitis.

Riwayat sosial :

Ia tinggal di sebuah rumah dengan 3 kamar tidur yang dibangun di atas plat betondimana ia tidur bersama dengan teman sekamar. Ia tinggal disana selama sekitar9 bulan. Salah satu teman sekamarnya adalah seorang perokok tetapi tidakmerokok dirumah. Angele terkadang merokok sesekali ketika ia minum alcohol.

Dia minum 5 atau 6 botol minuman sekali atau 2 kali seminggu. Salah satu temansekamarnya memelihara kucing di dalam ruangan. 8 bulan yang lalu Angele mulaikuliah di State University dimana ia kuliah keperawatan. Ia mulai bertanding sepakbola dan basket di universitas dan mengikuti kejuaraan local dan regional. Iamengaku dirinya tidak melakukan hubungan seksual secara aktif namun ia mulaimempertimbangkan untuk berhubungan seksual semenjak 7 bulan yang lalu.

Riwayat pengobatan :Angele sudah menjalani terapi Tavist 1 tablet secaraperoral BID (sehari dua kali) dan oxymetazolinenasal spray PRN pada malam hari (sekali atau 2 kaliseminggu), butterbur extract 1 kapsul peroral BID(sehari dua kali). Angele menderita alergi terhadapkodein karena menyebabkan gatal-gatal.
